Dr. Jefferson Holm PT, DPT, NCS – Owner
Founded Advanced Neurologic Rehabilitation after recognizing a need for expert neuro rehabilitative care in Arizona. Over the past several years, Dr. Holm has worked closely with several innovative rehabilitation companies gaining valuable experience working with adults who have suffered traumatic brain injury, stroke, spinal cord injury, Parkinson’s disease and Multiple Sclerosis. This experience has taught Dr. Holm that expert neurologic care requires a team of highly qualified, innovative, and competent individuals. Dr. Holm has been recognized by the American Board of Physical Therapy Specialties as a Neuro Clinical Specialist and by the American Physical Therapy Association as a certified vestibular therapist. He is also a certified provider in the LSVT BIG program for Parkinson’s treatment. Dr. Holm has been married 10 years to his beautiful wife Atrena and has four amazing children. When Dr. Holm is not at work, he enjoys swimming, running, spending time with his family, and serving in his church and community.
Jake Bishop, MOT, OTR/L
Jake Bishop graduated from ASU with an undergraduate degree in Exercise and Wellness and earned a Master’s in occupational therapy at AT Still University in Mesa, AZ in 2011. His clinical OT experience spans many settings including acute hospital care, sub-acute rehab, skilled nursing, and home health. In these settings he has gained experience working with a myriad of diagnoses and uses this experience to provide insight into how therapy can benefit a person as a whole, not just physically. He has been living in Gilbert since 1999, married his wife Karen in 2007, and has four children. Jake is a certified provider in the LSVT BIG program for Parkinson’s treatment.
Lauren Crane, CF-SLP
Lauren holds a Master of Science degree in Clinical Speech-Language Pathology from Northern Arizona University. She has experience working in a variety of settings including inpatient rehabilitation, outpatient rehabilitation, skilled nursing, and home health services. Lauren has a passion for providing individualized, compassionate, evidence-based patient care. Special areas of interest include aphasia, dysarthria, dysphagia, speech, apraxia of speech, and voice. Lauren is a certified Lee-Silverman Voice Treatment (LSVT) clinician. In her free time Lauren enjoys hiking, traveling and trying new foods/restaurants.
Nikki McCants, PT, DPT
Nikki graduated from the combined Doctorate in Physical Therapy program at University of California San Francisco and San Francisco State University with an interest in pediatric and adult neurological rehabilitation. She has experience working in a variety of clinical settings including pediatric acute care, pediatric inpatient rehabilitation, pediatric and adult neurological outpatient rehab, home health and early intervention. Nikki has experience working with a large variety of diagnoses and believes in a client and family-centered interdisciplinary approach to care. She is a native of northern California and has recently returned to the desert of Arizona.
